4th & 5th Grade Self-Release

This form is required for 4th and 5th-grade students who will be leaving the designated area on their own after service. By completing and signing this form, parents or guardians give permission for their child to check out independently. This ensures we have accurate records and follow proper safety measures.

At this age, many kids are ready for the responsibility of checking themselves out. As a parent, you have the option to allow your 4th or 5th grader to leave on their own at the end of service. If you choose this option, your student will be dismissed to meet you at a pre-arranged location. If you prefer to continue with parent check-out, your child will remain in the designated area until you arrive with the required security tag.

Every child will still need to check in as usual. We hope this option makes pickup smoother, especially for families with multiple children in different areas.
